2. SERVICE AREA AND ELIGIBILITY
CoastLift Medical Transport is headquartered and primarily operates from the State of Georgia. CoastLift provides non-emergency medical transportation services within approved areas of Southeast Georgia and may provide transportation to and from approved locations in North Florida.
Transportation outside CoastLift’s standard service area may be available on a case-by-case basis and is subject to driver availability, vehicle availability, scheduling, applicable licensing or permitting requirements, insurance coverage, operational capacity, and CoastLift approval.
CoastLift will operate in accordance with applicable federal, Georgia, Florida, county, municipal, and other local laws, regulations, licensing requirements, permits, and transportation requirements that apply to the specific transportation service being provided.
CoastLift reserves the right to decline, modify, reschedule, or cancel a transportation request when the requested service cannot be safely or lawfully provided or when applicable licensing, permitting, insurance, operational, or regulatory requirements have not been satisfied.
3. BOOKING AND SCHEDULING
Booking a Ride
Transportation may be requested through CoastLift’s website, online ride-request form, telephone, or another CoastLift-approved booking method.

CoastLift may request additional information before confirming the trip.
Advance Scheduling
Passengers are encouraged to schedule transportation as far in advance as reasonably possible. Advance scheduling improves the likelihood that CoastLift can accommodate the requested pickup time, vehicle type, mobility needs, and return transportation.

CoastLift understands that medical appointments and personal circumstances may change. Passengers are asked to notify CoastLift as soon as possible when transportation is no longer needed.
Cancellation, Rescheduling & No-Show
Transportation may be cancelled or rescheduled by calling 912-464-3780 or emailing info@coastliftmedtransport.com. Cancellations made at least 24 hours before the scheduled pickup time will not incur a cancellation fee.
Trips cancelled after a driver has been dispatched or after the driver has arrived at the pickup location may be subject to applicable cancellation or No-Show fees. Current No-Show fees are $50 for Ambulatory $65 for Wheelchair transportation and $125 for Broda® Traversa Chair transportation.
Healthcare provider appointment cancellations made before the driver arrives are generally exempt from cancellation fees.
Drivers typically wait up to 15 minutes after the scheduled pickup time before a trip is considered a No-Show unless other arrangements have been made.
Please refer to the complete Cancellation, Rescheduling & No-Show Policy available on our website for additional details.

Estimated pickup or arrival times are estimates and are not guarantees unless CoastLift expressly agrees otherwise in writing.
CoastLift is not responsible for missed appointments, delayed appointments, lost appointment opportunities, or related expenses resulting from circumstances beyond CoastLift’s reasonable control.
If CoastLift becomes aware of a significant service disruption, it will make reasonable efforts to communicate with the passenger or booking representative and, when feasible, provide an alternative arrangement.

16. GOVERNING LAW AND VENUE
These Terms are governed by and interpreted in accordance with the laws of the State of Georgia, without regard to Georgia’s conflict-of-law principles.

To the extent permitted by applicable law, any legal action arising from or relating to these Terms or CoastLift’s services shall be brought in a court of competent jurisdiction in the State of Georgia. CoastLift’s services may be subject to different federal, state, county, municipal, healthcare-program, insurance, or contractual requirements depending on the passenger’s pickup location, destination, payer source, service type, vehicle, and applicable jurisdiction.
Nothing in these Terms creates a representation that CoastLift is authorized to provide a service that requires a license, permit, certification, contract, or approval that CoastLift does not hold. CoastLift may decline or modify a transportation request when necessary to comply with applicable legal, regulatory, licensing, insurance, or safety requirements.
20. SEVERABILITY
